All patients are registered with the practice rather than with an individual GP. However, all patients are also assigned to a list which is managed by an individual GP. This is your named accountable GP. We will sometimes refer to this as your usual GP. Your named accountable GP is responsible for co-ordinating your care and this is the person to whom any correspondence or test results relating to you will come to in the first instance. If you are unsure who your named accountable GP is you can find out by contacting the surgery. If you wish to change your named accountable GP at any time please contact the surgery.
|Following birth, Community Midwives will provide care before handling over to Health Visitors for further infant care and advice. Postnatal maternal and baby checks are conducted by doctors at Charlton Hill Surgery approximately six weeks after the birth. |
