NEW PATIENT REGISTRATION
Enquiries regarding registration can be made by telephone or in person to Reception. Staff will advise on the procedure to be followed. New patients will be required to come into the surgery and complete the required forms. Registration will only be accepted with valid and current ID including proof of address.
Copies of the registration form can be downloaded here. Please ensure that you use the correct one. You will need to return the form to the practice together with your ID.

Once form is completed please email to email@example.com or post through letterbox (behind hedge on left hand side of front doors). We do not need to see ID during the Covid-19 period.
The practice area is contractually agreed with West Hampshire Clinical Commissioning Group. Practices can refuse patients onto their lists if the patients address is outside the practice area. Our agreed practice area is the SP11 postcode area (except Wiltshire). Despite being located in SP10, SP10 is not in our agreed practice area.
However, regardless of this, the practice is currently accepting new patients from the SP10 postcode as well as those resident in the SP11 postcode area (except Wiltshire).
Since 1 April 2015 patients have been able to register with any practice regardless of whether they live in the practice area, if the practice will accept such patients. Charlton Hill have decided that we will not accept patients from out of area, with the exception of those patients residing in SP10. This is because of the issues around ensuring that home visiting services and community nursing services are available in the area in which the patient resides, and clinical responsibility falling back to the GP to arrange these services should a scheme local to the patients home not be in place.
